Important Athlete and Parent Forms


2015-2016 sports physical form


Bring the physical form filled out and signed by your parent or guardian (available from school activities office or by clicking on the link above office). No exam will be allowed unless signed by a parent. Be sure immunization updates are noted on form (especially last tetanus and MMR). No immunizations can be administered at this time.

These are sports physicals and do NOT take the place of a comprehensive physical. No immunizations will be given.
The form must be filled out and turned in before students may participate in athletics. Includes required CONCUSSION information that must be filled out.

State Qualifiers at the 2016 State Track & Field Meet

2016 State Meet Success


We had a successful conclusion to our 2016 season with a great group of athletes who had some of their best performances at the state meet.


We qualified 11 entries in 11 events and 24 athletes to the state meet. We finished 24th with 6 points. We also placed two events in the top 5 of all time:


The 4 x 100 team of Olivia brooks, Peyton Daugherty, Kelsey Fischer and Mikala Sidney’s state finals time was the 2nd best all time in AHS history and the 4 x 200 relay team of Kaitlyn Mohrfeld, Peyton Daugherty, Kelsey Fischer and Mikala Sidney ran the 5th best time in AHS history at state.

TEAM MEALS FOR ALL TRACK PARTICIPANTS:

March 31st, April 5th, April 12th, April 26th , May 3rd (5:00-6:00 PM)

Team meals are a very important part of the bonding experience for all track participants. This is a time when the sprinters, distance and field girls mingle and come together to get to know one another while enjoying a meal. This has been a Hawkette tradition and to keep it going we need everyone to sign up to help bring food. Please sign up to bring food for 2 out of 4 meals and make sure that you are signed up for at least one main dish. There will be a sign up genius that goes out the week prior to the meal to assure that the girls have enough food but we need confirmation from all of you when you can help at the beginning of the season. Please feel free to come help serve the meals and clean up after.

HAWKETTE INVITATIONAL:

April 6th (4:00-9:00 PM)

We can’t do this without the help of every family. We have spots to fill for field events, concessions and other tasks! This is also Senior Night and we try to make sure that the Senior parents are for the most part free to watch their daughter and participate in the Senior Night activities.

BOOSTER BASH:

Saturday, April 29th (5-9)

This is a dinner/auction fundraising event for all of Ankeny. The proceeds go directly to the Athletic Departments to be used on our athletes! It is a very fun event in which you can get together with your friends for great food and entertainment for the benefit of your children. To sign up to attend the event or make a donation go to http://www.ankenyboosters.com. We would love to see a table or two full of Hawkette Track parents!!! Beth would be glad to answer any questions!!

TEAM MEALS FOR STATE MEET:

May 17th

This team meal is for the qualifiers and the food will be provided by the qualifiers parents. We request that when you daughter qualifies you will sign up to bring a food item to the meal. These meals are incredible for the girls with lots of emotion and bonding!

DOOR SIGNS:

Saturday May 12th, (10am-Noon)

When your daughter qualifies at the Conference Track Meet on 5-4-17 they will be going to the State Track Meet! We will be making door signs for your homes front door on Friday 5-5-17. We will have everything that you need and will be set up in the hallway behind the gym at Southview from 4:30-6:00 PM. Normally the parents of the runners come and do these signs for their daughters that qualify. Please mark this date on your calendar and if you are not able to come please ask a grandma, special friend or relative to come and make the door sign. PLEASE call Jennifer if you have conflicts or questions.

AWARDS BANQUET:

May 24th (6:00-9:00 PM)

Please make sure to come and enjoy this meal and presentation of the Track and Field season accomplishments and video provided by Coach Adamson (bring some Kleenex)! This is an event to bring your entire family! Please bring $5 if you would like a DVD of the season (what a great memory!) Every family will bring 2 servings according to their last name:

The Drake Relays are April 27-29.9.


Make sure you keep the May 5th and may 12th open in case the conference or state qualifying meets are meets are postponed.


We will have practice for those who qualify for the state meet on Saturday, May 13th. We will also make door signs for the state participants at this practice.


The State meet is May 18-20 at Drake Stadium


The Track Awards Banquet will be held Wednesday, May 24th at 6PM.

Make sure you read pages 29-40 in the AHS/ Southview Student and Parent Handbook, which is online at: http://www.ankenyschools.org/Page/3620  It has detailed information each parent and athlete should know.  An important part of this policy is the anti-bullying and hazing language.
